
Since COVID-19, the applied use of Multidisciplinary Team coordination in the community has risen on the agenda. Local authorities and NHS teams have turned to MDTs as a way to align services. Initiatives like the Proactive Anticipatory Care (PAC) Model are part of this movement.
But many of these approaches remain institutionally led. Coordination happens between organisations, not with the community.
