CareConcierge can help if you are struggling to access care when you need it, or if you have been sent for treatment and you spend a lot of your time frustrated, waiting, rushing and not understanding.

CareConcierge (CC) makes it easier to get help and organise support to care for yourself and others. Whether you’re a patient, carer, or concerned family member, you can contact your local team. They will help you understand the next steps and cut through the admin by provding Health and Social care organisations with your latest information. CC can help to answer questions like:

  • What service do I need?
  • Who has the information and where can I find it?
  • Who can I speak to?
  • What happens next and what do I need to do?

CC’s team understand how health and social services work They will follow the treatment pathway to connect organisations, help you gather information and ensure it’s with the right carer at the right time. If there is a delay or something is not clear they will help you with calls, find missing information and help you to plan.

We are working with residents and care providers to design CareConcierge, we already plan to include:

🔵 A helpdesk that residents, patients and caregivers can call
🔵 An app for carers to manage their information, contact care providers and coordinate care
🔵 An app for organisations to improve their service; they will be able to create neighbourhood care pathways that others can follow
🔵 A team that can monitor and help organisations.

Different care providers and trades will be able to use CC to co-ordinate activity as a single team, reduce costs, improve capacity and quality. Patients and their caregivers will find it much easier to speak to someone who can help and manage information and activity with their carers, making it simpler to know what to do next for a smoother journey.

We are starting with a pilot to improve coordination across health, social care, and community services across SW London. I am donating Ardeo’s eMDT as a foundation to help CC establish itself. It will address rising demands, workforce pressures, and care fragmentation. The integrated helpdesk has tools that will enable professionals, families, and service providers to cooperate effectively, reducing care missteps and delays.

The CareConcierge platform is open to encourage NHS, Social Care services, independent providers and health trade suppliers to work with us. It can be used now to set up neighbourhood level MDTs, prioritise urgent needs, and help organisations to improve their pathways.
More importantly residents and caregivers can communicate with their providers, access up to date care plans and help coordinate support.
